Registered Nurses (RN)

Registered Nurses begin the patient's care plan and decide visit frequencies in collaboration with the MD. RNs administer and monitor medication, and medical equipment, and consult with the patient/family on any concerns or information regarding the patient.

Medical Director (MD)

Medical Directors oversee and manage the care given. They coordinate all the clinical staff to ensure they follow the care plan set out.

Every 2 weeks, the MD and clinical team come together to discuss each patient's personal care plan. At this Inter-Disciplinary Team (IDT) meeting, they discuss possible changes to medication, order tests, or review and continue the care plan for each patient.

Medical Social Worker (MSW)

Medical Social Workers provide all of the legal and economic care for the patient and family. This includes post-life wishes such as mortuaries and transportation logistics. MSWs may also assist with any government documents needed.

Certified Home Health Aids (CHHA)

Certified Home Health Aids provide any and all grooming needs for the patient. The CHHA follows the care plan for the patient- giving baths, combing hair, and changing bed sheets per the patient's needs

Chaplain

Chaplains provide spiritual care for the patient and family per request. They will respect all  views, religions, and perspectives the patient and family may have. Chaplains will also help coordinate post-life bereavement if needed.

Licensed Vocational Nurses (LVN)

Licensed Vocational Nurses assist the RN in visiting the patient during the week. LVNs visit after-hours when requested due to a change in the patient's condition.
